Abstract: Spillway Tainter (radial), gate No. 3 failed at Folsom Dam, California, on July 17, 1995, in the early morning. The failure resulted in a discharge of 1,132 m^3/s into the lower American River. This paper will address how the forensic team formed to investigate this failure functioned, and arrived at a failure theory.
